Talking about the health and emotional issues people face after a stroke can be difficult.
Stroke Touchpoint Cards are designed to facilitate person-centred conversations between patients and healthcare professionals. The patient and the staff member both choose cards that represent issues they wish to discuss. These cards give ‘permission’ for those difficult issues to be discussed when they might otherwise be avoided.
They help to make appointments more effective for both patient and healthcare professional. The cards help focus on the following broad issues: • Health • Effects of Stroke • Feelings • Daily Living
Suitable for healthcare professionals working with stroke patients; the cards can also be used to inform family members. 1-1 activity session requires a professional with knowledge of stroke aftercare. 20-30 mins.
